What documents must PwBD candidates upload for reservation?

A valid UDID card issued by a designated medical authority under Ministry of Social Justice and Empowerment (MoSJE), and must appear before the Medical Board constituted by the Competent Authority.

Are OCI/PIO cardholders registered on or after 04.03.2021 eligible for Indian citizen reserved seats?

No. They are eligible only for NRI seats or supernumerary seats, not for seats reserved exclusively for Indian Citizens.
